Select appropriate application to more effectively manage their Resource Connection Gateway program. Gaston County Family YMCA would like assistance in reviewing and formalizing their process and in helping to find and select an application to manage this program more effectively as well as give them access to better reporting and outcomes.

Project Description

Gaston County Family YMCA started a program to help end the cycle of poverty in their community through the creation of a Resource Connection Gateway.  The Resource Connection Gateway provides a single connection point to resources to address these and other human needs. The Gateway extends a hand-up and not just a hand-out to anyone in need. It acts as a bridge between people in need to organizations that can help them with their specific need such as MedAssist, WIOA, Goodwill Job Services, Labor Finders and crisis agencies/food sites. They have had tremendous growth in this program since its inception 2 years ago.  They have between 1-200 referring agents and to date have had over 1200 unique individuals referred.  They have a 4 hour turnaround time from when a referral is made to when a call back is made. Currently they have one person managing the process of receiving the referrals and making the call backs to individuals.  With continued growth, their current process will be increasingly difficult to sustain and will create risk for burn out.  They would also like to have a better way to collect data and report outcomes to potential funders/grants as well as track referred individuals over time (i.e. what happened to them later?).  Their program has also received attention from the national YMCA organization and would like to be able to replicate the solution throughout the entire Y organization.  They would like assistance in reviewing and formalizing their process and in helping to find and select an application to manage this program more effectively as well as give them access to better reporting and outcomes.  They currently have a link to make referrals off of their website – all of the information is collected in a Google Doc and is emailed to the individual managing the process.
